User unable to make decision

Using version 11.5.      I need help ugh

I created a process with an automated decision that brings the user to a manual decision with a Y/N answer.  For some reason, they can say no but cannot choose yes.  When they choose yes, the error below is what they get.  I went to the sub-process and the next stage is an automated email action which then goes to an auto-decision.  I’ve provided the two processes to show you.

 

First process with auto decision

Troubleshooting done:

I have checked the SVD to ensure they have the ability to use Decision Required, I have checked the user to ensure she is in the right SVD to make the decision, I checked the Action Type to ensure SVD can use it.  Everything seems ok. 

Got another user from that SVD to try and same error.  We assigned it to another user from a different SVD and he was able to choose yes.

Also checked to see if they can make other decisions from other processes and they can.

Are there any expression on decision task’s assigned user field which refers a custom field?
How is the “automated email action” configured in the process designer?

The expression on the automated Decision: Is the user a contract employee?:

$new.parentEvent.W("USER_CONTRACT").shortCode="YES" (contract user)

$new.parentEvent.W("USER_CONTRACT").shortCode="NO" (government user)

 

On the form, when they choose No (not contact) it goes to the second decision, where they can’t make the Yes decision:

UPDATE:  I think its the stage that is the issue for them.  I changed the Controlling Action from RFC Stage to Normal and she didn’t get the error but… the process got stuck there and didn’t continue.  I changed it back to RFC and she got the error.

​​​​​​-Seems like there are more criteria we need to check regarding this issue. 
-Since the user can take answer no, I guess the issue should be with the first task after the answer yes.
-Can you check whether the user/user svd has privileges to take the RFC stage action from the next task after the answer yes is selected ?

-If that did not work, then can you log an incident with support so we can do a proper investigation ?
