Can someone tell me how the Push Enabled box gets checked for mobile users? On this screen it is read-only.

Is this same for all users for only for you ?
This filed is read only and it set during app activation. Could be couple of reasons.One is when App install on your device, you may see a message to allow notification. You may miss that ..

May be you disable/delete the device. Then reactivate as a new device and check
