I am facing an issue where once an attachment is selected from the Request screen, the file path, name, description disappears after clicking the “Save” button.
The values are passed to the DB as well.

How can I fix this?

This is not a baseline behviour.
Check if there is a client script or business rule running after the save action.

This mainly happens when the corresponding metadata tables have invalid relationships on the custom metadata level and ui level. Do check for incorrect relationships and also make sure that there are no additional restraining constraints such as invalid scripts /br.
