I think you can use the predefined events for our file operation logging for this. Every read, write and delete operation is logged and have it's own event.

I created a Custom Action Event using Event : Document Approved that send Notif to the group of persons declared  (with Administration,Edition Access Level) fwhen the document is approved.

New revision is : created (own Event for Every read, write and delete operation)
