Is there a setting to default new document attachments to “Approved” and “Released” status when registered? To change the document status so that it can not be edited without performing a formal revision cycle.

For this requirement you can use this On Save - Release flag,
+32Note: The above feature exists only in that screen. If you want new document attachments created under Attachments / Documents to be approved and released automatically, you will need to develop a custom event or workflow to do that. There's no built-in support for it.
