Failed executing query (IO Error: Checksum fail)

Got the error message above when openning Create user. Any idea?

This is IFS9 with Oracle Database . 

We have tried reconfigure the middleware, restart server, restart db instances. None of them resolve the issue.

yes, it was logged in as ifsapp

I recommend you create a high importance case in the support portal to get IFS to engage some support resources to assist you.
