The issue likely that the mobile service user account connecting to FSM Server has likely expired or password has changed.
Check the DownloadUrl of the mobile Service Web config file is correct and downloadable it by outside of the network.
Go to custom metadata of your custom table and check for Table Namespace. And check the customassembilies are having an entity for the table with correct name space, and it is correctly referred in FSM Server Web config in ApplicationExtension section.
Check the Class and method signature is matching with the MPM you configured in Perform Definitions. And check whether the class the MPM method contains is public.
