Maintenance Survey Answers not showing WO and Task Sequence

Hi All, We are currently using MWO Service 10 (UPD 9) and we use the Maintenance Survey functionality by having E-forms triggered when se use the ‘Start Work’ button after accepting Work from the Pool Work. And we have noticed that some Answer IDs are there without a Task Sequence and WO number in the DB side and the Survey Answer page. Would you kindly advise if that could be fixed?

Hi @James Ashmore Thanks for your response. In addition to above we are experiencing that the Maintenace survey is triggering multiple times even its not intended in Workflow Configuration. And this occurs intermittently. By any chance if you all have any experienced the same and would you kindly suggest if there is a way to solved that and is there any connection between above previously mentioned issue?
