I have a media item setup in an equipment object.
I want to change it.
I have deleted the media item, but the previous media item is still visible and I cannot change it.

Quick clarification, when you say you deleted it, did you do it from the media items page or did you remove the media item connection from the equipment object?
Also can you still see any records in the media library of this equipment object? (You can check this by clicking manage media in the attachment panel media section)
